Revision BBL Spotlight: Redo BBL, Rib Remodeling & Custom Implants

In this Surgical Spotlight, we present a highly complex revision BBL case. This patient had already undergone two previous BBLs elsewhere, resulting in a very large, “baroque” buttock volume but a square, unsculpted silhouette. As is common in revision work, we were facing extensive prior liposuction, deep fibrosis and very limited usable fat.

To transform this starting point into a truly sculpted figure, we combined three advanced techniques: a redo BBL, rib remodeling to refine the waist, and custom buttock implants to restore projection and shape in a controlled, predictable way.

Why Revision BBL Cases Are So Challenging

  • Multiple previous surgeries: 2+ prior BBLs with aggressive fat manipulation.
  • Deep fibrosis: scarring and tissue stiffness that make shaping and fat placement more difficult.
  • Limited donor fat: little to no usable fat left after previous liposuction sessions.

  • Custom buttock implants: bespoke implants to provide a stable, predictable core of projection where fat alone was insufficient.
  • 3D contour integration: blending implants, fat and waist work to create a smooth, continuous, sculpted silhouette.

The Result

The transformation is intentionally dramatic: a move from an overfilled but unshaped backside to a sculpted, hourglass silhouette with defined waist, enhanced hip curve and controlled, elegant projection. Instead of chasing size alone, the focus is on shape, proportion and balance.
